Compass Health Care Inc. is committed to providing specialized housing services to individuals being released from incarceration. Women transitioning from SACRC to Compass LIBERTAD project will initially be enrolled in level one: Identifying and accessing community resources. Individuals who complete level one will graduate to level two semi-supervised housing.
Level one is designed to empower individuals to identity and learn to access appropriate resources within the community such as supports in recovery, 12-step groups, mentors, health/dental care and maneuver through public health systems.
Completion of level one will be demonstrated by the individual’s attendance at 12-step meetings, obtaining health/dental care and commitment to community organizations. Participants will graduate level one and be transitioned to a semi-independent housing unit.
